Please make sure the following forms are filled out on Genesis

-Steroid Release -Concussion Release

-Sudden Cardiac Release -Opioid Release -NJSIAA Steroid Test Release

-Random Drug Test (RDT) Release -Athletic Contract Release

You must have an up to date physical submitted to Mr. Lasala prior to the start of the season. Physical Forms can be found on the Athletic page
