1) Forms that must be completed and returned to Coach Csapo. 

--This form can be placed in Coach Csapo's mailbox at CHS or given to Coach Csapo the first day of practice.

• Emergency form - Complete the emergency form and return it to Coach Csapo the first day of practice

• Health History Questionnaire - Must be completed and given to your doctor with the Physical form, then handed in to your COACH. 

2) Forms that must be completed and returned to Mr. Grasso, the Athletic Director at CHS. 

• Physical form

  • Supplemental Physical Forms (In case your doctor requests these)(  in case your doctor requests these

• The PHYSICAL  FORM MUST BE SUBMITTED BEFORE ATTENDING ANY PRACTICE. Please return the physical form to Mr. Grasso, the Athletic Director at CHS.  Physical forms must be printed out and submitted (no electronic copies or email attachments will be accepted).

• A physical is also required for participation in a sport. Physicals are good for 365 calendar days. Athletes must print out the physical form and make sure your physician stamps the four pages somewhere on the inside portion. PHYSICALS MUST BE SIGNED, STAMPED AND DATED BY YOUR PHYSICIAN OR THEY ARE INVALID!

• Physical forms are specific and need to be NJSIAA approved. A physician's personal physical form cannot be accepted.
