Physical Information

Please click HERE  for the link to the school Nurse's google site.  All Physical information can be found under the "Athletic Forms" tab. Once the physical form is completed by a doctor, it must be returned to school, then cleared by the Nurse and district Doctor. To ensure enough time for physicals to be cleared, they must be turned in to the Nurse by February 24th, 2025 

Physical forms can also be printed out of picked up at the nurses office.

You must have the following in order to tryout:

• CLEARED Physical (by doctor and school nurse)

• (Health History Update Form)

• Concussion Form

• Sudden Cardiac Pamphlet Sign-Off
