Student Chromebook Agreement (READ AGREEMENT BEFORE SIGNING)

These students are required to have new forms filled out and signed by a parent or guardian:

• All 9th graders (return your middle school device to North Forsyth in order to get your new Chromebook)

• Day Users

• Students who are new to Winston-Salem/Forsyth County schools

Important Info on the "OPT-OUT" option:

Please note that every student is required to have access to a school-issued Chromebook due to state testing requirements. However, if you or your student wish to not take a Chromebook home then they may sign up to be a "day user." Their assigned Chromebook will remain at school in the library. They will be required to show ID when they come to pick up their Chromebook, and they are responsible for returning it to the library at the end of the day. A signed agreement is still required of all Day Users.
