Re: Medication Policy


I would like to take this opportunity to briefly review the policy for medication administration at SRES. Any student taking a prescription medication who may require a dose at school must have a signed form stating the name of the medication, dose, and reason for the medication. This form will be kept in a confidential health record and must be signed by the physician as well as the parent/guardian of the student. Any student requiring a prescription medication during school hours must see the nurse for his/her dose and may not carry the medication to school unless specifically requested by the student’s physician. All medication must be provided in a medication bottle with the student’s name and correct dosing information. Medication prescription forms are available on the Health Office link of the SRES website, or at the SRES Health Office as well.


The students who may require a specific non-prescription medication will also need to have a form signed by the parent/guardian and the dose will be administered and documented by the nurse. Again, no medication whether prescription or non-prescription should be given to students to carry to school unless specifically requested by the student’s physician.
