Welcome to the FAS CLUBS AND ATHLETICS Website

In order to try out for ANY SPORT, the following 3 tasks MUST be complete: 

• the appropriate Google Form is filled out (to the left of this page)

• a Health History Questionairre is filled out (this must be done EVERY SEASON-copies found on the Sports/Open Gym Sign up tab to the left)

• a CURRENT PHYSICAL is on file with the nurse (that does not expire during the season-copies found on the Sports/Open Gym Sign up tab to the left )

Due dates for each season are discussed during preseason team meetings.  FOR THE WINTER OF 2026-2027, ALL PAPERWORK MUST BE COMPLETED/RETURNED BY TUESDAY, NOVEMBER 10th.
