If this is your first time filling out a final forms account:
your parent needs to create an account first, they tell the system who their kid is and then you can make yours
Your parent's account tells the school what sports you are doing. Selecting all sports for the entire year will get you on the right rosters so you do not have to worry about it later.
Every year after you make your account you will need to fill out a couple of sections, but by and large this is a one and done type of thing.

In addition to the final forms sections you will also need to get a physical done.
There are forms on final forms as well as in the front office (most doctors offices have forms as well and are used to this process).
Once you have a physical form signed you can upload it yourself to final forms or bring it to the front office and they can upload it for you.
