This site was created to provide information on the secondary transition process and includes an associated Google Classroom platform to accompany the material. A certificate of completion (including hours associated with each section) will be issued upon completion of assignments.

  • If you are experiencing difficulty accessing the Google Classroom, it may be due to your district network settings. Either speak to your IT department about accessing out of district network Google Classrooms or use a non-district email to access.

For convenience links to each sections assignment and assessment is located at the end of each section. You must register for the classroom prior to accessing these links.

Secondary Transition

Secondary Transition is the process of preparing students for their lives after high school. In Pennsylvania, transition planning begins at the age of 14 (or earlier if determined by the IEP team).

IDEA 2004 defines secondary transition as “a coordinated set of activities for a child with a disability that is designed within a results-oriented process, that is focused on improving the academic and functional achievement of the child with the disability to facilitate the movement from school to post-school activities, including post-secondary education, vocational education, integrated employment, adult services, independent living or community participation.”

At the secondary level, transition is the component that drives IEP development.
