Form Requests

1. Request Bragg Sheet from your counselor

2. Complete form

3. Give a copy to your designated counselor and/or teachers when you request a letter of recommendation.

Please allow a two week minimum for a letter of recommendation to be completed.

1. Request Transcript Release form from your counselor

2. Hand in to your school counselor at least two weeks prior to the due date.
