REMINDERS

Before 7th grade students can begin the school year, they must submit updated shot records documenting their updated TDAP vaccination.  

If you would like to view the religious exemption waiver, please click here. This must be provided before the start of next school year. 

You can submit the TDAP shot record or exemption paperwork using the following methods:  

• Turn it into the main office (Summer office hours: 8am - 2pm and closed for lunch 12pm - 1pm)

• Fax it to the MJMS Office  (615) 754-7566 (Attention: Missy Briney)

• Email the information to brineym@wcschools.com
