Guidelines for Use of Animals and Humans in Student Science Projects
If your science project involves humans or animals in any way, you must read these guidelines and complete the necessary forms.
Animal (Vet) Forms
Students working with any non-human living organisms that are not plants need to complete Form 1 and Form 2. Rules and guidelines for working with animals can be found in this document. These forms are required by both the Willits and Mendocino County Science Fairs.
Informed Consent Form
Students that collect data from or about people must obtain the participants' permission. All people who volunteer to participate in a science project must sign this form. The student is responsible for ensuring forms are signed. Projects that participate in the Mendocino County Science Fair may be asked to provided copies of all signed Informed Consent forms.
Human Research Compliance Form
Students who use human volunteers or collect data in any way about humans must complete these forms. The student's teacher (or advisor) is responsible for approving the project. This form is required for both the Willits Science Fair and the Mendocino County Science Fair.
Human and Vertebrate Tissue Form
If your project involves the use of human or vertebrate tissues, you must complete this form. Students are not allowed to be involved in the direct acquisition of living tissue from humans or vertebrate animals. This form is required for both the Willits Science Fair and the Mendocino County Science Fair.
