The following documents need to be turned in to the Student Services Office at least 2 weeks before you want to submit an application for two and four year colleges and/or tech schools. Paper copies of all forms are available in the Student Services Office. Scroll down to see other paperwork you may need to submit with your application.

Required Paperwork for Four Year College Applications
The following documents need to be turned in to the Student Services Office at least 2 weeks before you want to submit an application that requires a recommendation from your counselor. This includes the majority of 4 year colleges. Paper copies of all forms are available in the Student Services Office.
**If filling out forms in Chrome, do not choose "save as...," choose "Print..." and select "Save as PDF" under destination or printer. If you download the file and open in in Adobe you can save the file.**
Have a parent fill out this form and return with your other paperwork to Student Services so your School Counselor can write you a better recommendation
Fill out this form and return it to your School Counselor so they can include everything important about you in their recommendation
Your counselor will use this sheet to write recommendations and potentially choose scholarship recipients
Give this completed to a form after a teacher agrees to write a letter of recommendation for you
