This form will provide the information needed in choosing award recipients so please be thoughtful and thorough in your responses.
The resume section in WITS must be completed BEFORE you submit this form. Without both the completed resume and the self-nomination form you will not be considered for an award.
This form is due by March 14, 2025 at 3pm. Late applications will not be accepted.

Bausch & Lomb Honorary Science Award: This is awarded to high school juniors who demonstrate high achievement and rigor in science and math classes, with positive contributions to their school and within the larger community. The nominated student will have their application fee waived and be eligible to be considered for a merit scholarship of at least $5,000 a year.
Students who are interested in this scholarship and feel they meet the criteria must complete the Google Form by March 11th at 3pm. Your WITS resume must be completed for consideration. Students with incomplete resumes will not be considered.
Frederick Douglass & Susan B. Anthony:This is awarded to high school juniors who demonstrate commitment to understanding and addressing difficult social issues, who shows leadership and dedication to community action, has strong grades and rigorous courses taken in the humanities and social sciences. The nominated student will have their application fee waived and be eligible to be considered for a merit scholarship of at least $5,000 a year.

George Eastman Young Leaders Award: This award recognizes high school juniors who will be chosen on the basis of outstanding achievement in leadership & academics, high school grades and challenging courses, and extensive involvement in extracurricular activities. The nominated student will have their application fee waived and be eligible to be considered for a merit scholarship of at least $5,000 a year.

Xerox Award For Innovation & Technology: This award recognizes high school juniors. The award criteria is
*A Strong interest in innovation and/or information technology, and a high level of achievement in this area
* Exposure to new technologies outside of school, such as pursuing serious work opportunities in local laboratories or industry
* Leading other students to new approaches to old problems; business plans for fundraising; innovative community service; after-school activities; and best uses of communications tools such as chat and blog. The nominated student will have their application fee waived and be eligible to be considered for a merit scholarship of at least $5,000 a year.
