Newborn Appointment Scheduling
You will usually call our office before your baby is discharged from the hospital to set up their first appointment. When you call, we will ask for the following information:
Baby’s full name
Date of Birth (DOB)
MRN# (found on the baby’s hospital bracelet—usually on the wrist or ankle)
Home address
If your baby has siblings who are already patients here (be sure to let the receptionist know)
Which hospital you delivered at, so we can fax and request the newborn records
If you are transferring to our office, please follow these steps:
Go to our Forms page and complete the New Patient Paperwork.
Go to Forms again and complete the Record Release Form — this is where you will list the office or provider you were previously seen at.
Drop these forms off at the front office. Once received, we will create your chart and request your records.
After we receive your records from your previous primary care provider, you can then call to schedule a Well Child Check.

Newborns
Please fill out and bring the new patient paperwork(found in the Forms tab) - if unable to fill out beforehand, you can fill it out before your appointment in office.
Insurance cards - you have 30 days to add your newborn to your insurance.
Patients Transferring to our office
Please bring your insurance card(s).
We should already have your patient intake forms
We should already have your record release form
If you have a previous balance on your account, please contact our office to make a payment by phone, or you are welcome to come in and pay in person
If you decide not to vaccinate your child, we respect your decision. We do require a vaccination refusal form to be signed to document your understanding of the associated risks.
Patients may request a medication refill using one of the following methods:
During the appointment: Ask your provider at the time of your visit.
By phone: Call our office during regular business hours.
Through your pharmacy: Have your pharmacy send an electronic refill request on your behalf.
Our office ages out at 20. We encourage you to begin looking for an adult primary care provider in advance before your 20th birthday to ensure a smooth transition of care.
