Parents/Guardians: Please print, date and have both you and your student print names and sign. Check either I DO or I DO NOT if you are consenting to exposure to animals.

Free Reduced Lunch.pdfWaiver of Student Fees:
If your student qualifies for free lunch at their home school please complete this waiver to be exempt from our $30 Vet Assisting fee. You may print, fax (815) 838-1163 or email back to us after completing the form.
