Student Checklist - A helpful document to ensure you have all your paperwork complete.

1. WCS Parent/Guardian Form: WCS introduction to the Harrington Job Shadow opportunity and permission to participate

Instructions: Print or receive a hard copy from Mr. Savage. This form should be signed and returned to Mr. Savage

2. Harrington Policies and Education Review: Information on hospital expectations regarding behavior, attire, professionalism, etc.

Instructions: To be reviewed by the student.

3. Harrington Student Onboarding Packet: The student application to participate

Instructions: Download, print, or receive a hard copy from Mr. Savage. This form is to be completed by the student and can be completed as a hard copy OR as a digital copy and returned to Mr. Savage

4. Harrington Immunization Requirements: Required form by the Harrington Healthcare System

Instructions: Print or receive a hard copy from Mr. Savage. A hard copy of this form will be signed by the student's healthcare provider or the school nurse (the school nurse will require adequate notice to review student records) and returned to Mr. Savage.

5. Covid Attestation of self Monitoring: Required form by the Harrington Healthcare System

Instructions: Print or receive a hard copy from Mr. Savage. A hard copy of this form will be completed by the school nurse or administration and returned to Mr. Savage.

6. COVID-19 Vaccination: Harrington Hospital requires participants to be fully vaccinated for COVID-19.

Instructions: Bring a copy of a COVID-19 vaccination card, or a statement from your physician that you have been fully vaccinated.
