Download a copy of the application form and fill it in with all the necessary details. Your submission must include the following: 

1. The completed scholarship application form (downloadable from this page)

2. Letter(s) of recommendation from a teacher and/or athletic coach by May 1, 2025

3. A true copy of the applicant's Ontario high school student transcript by May 1, 2025

4. Acceptance letter from an accredited post-secondary institution within Canada for a full-time degree-seeking program sent by May 1, 2025

5. A brief cover letter (approximately half a page) to give more insight into why you would be an excellent candidate for the scholarship. You can include your scholarly, athletic, work, and/or volunteer accomplishments and future goals, etc. by May 1, 2025

6. A recent student photograph by May 1, 2025
