Fill out this form and submit with your receipts to the Treasurer's mailbox in the school office to be reimbursed for PTA expenses.  

Deposit Form

Fill out this form for all PTA deposits.  Deposits can be place in the PTA depository safe in the school office.  Zippered pouches are available next to the safe.

Grant requests can be made by faculty and staff with this form.
