If you are interested in joining our team and would like to have all paperwork in place for when we are able to participate in person again, please download the medical form and release form below. Those forms can be scanned and emailed to soriwildcats@gmail. If you have any questions please complete the form below.
Our team participates in 3 athletic competitions throughout the year:
Fall - Bowling
Winter - Basketball Skills
Spring - Track and Field
Athletes must be at least 8 years old.
Below are forms that must be completed to participate in Special Olympics activities.

Medical Form must be completed by the athlete's physician. Medical forms expire 3 years from the date of the physical exam.
SORI_Release_Form_12.pdf
Release Form: This form must be completed by parent/guardian of athlete or by the athlete if they are older than 18 years old. This form must be completed every three years.
