Each student with an IEP also has a transition plan. This plan is intended to help guide students toward their future goals once they graduate from high school. The West Holmes Transition Website is a great resource for things like college and career research, ACT prep, and more. Many of these activities are required in the child's transition plan.

The parent questionnaire helps gather information relevant to creating your child's individual education plan. This online form can be filled out in place of the green paper copy sent home with your child.

Ohio Means Jobs Website

The Ohio Means Jobs Career Cluster Inventory is one tool to help students learn about their interests and possible future careers that meet these interests. It is also required of every WHHS freshman student. Students can set up their own account by clicking the "Sign In" in the upper right corner of the page and then choosing "Register Now" from the popup box. Students should fill in their personal information and then setting a username and password. This username should follow this formula: last two digits of graduation year, first initial, last name followed by "@wh.edu". For example, a username would look like this: 19asmith@wh.edu. The password should be set as the school ID code (the same used in the cafeteria to pay for food). Once registered, students can browse the pages and then complete the Career Cluster Inventory.
