If your child is new to Westfield School District:
*New Medical Record Form needs to be completed by Physician or NP (initially required when entering district and encouraged at each stage of growth & development)
*Health History Form needs to be completed by Parent/Guardian for admission to the Westfield School District
If your child is turning 11 and/or entering the 6th Grade and has NOT had their TDap & Meningococcal Vaccinations:
*6th Grade Required Immunization Parent Letter
If your child requires any medication during school hours:
*Please note the following Medication forms need to be renewed each school year and do not carry over*
*Medical Authorization Form needs to be completed, signed by you and your MD, and returned to the School Nurse with the medication.
If your child has a food or other allergy and requires treatment of Benadryl or Epinephrine:
These 3 documents need to be completed annually *Food Allergy Action Plan, *Epi-pen Parent Consent and *Emergency Health Care Plan for Designee
If your child has Asthma and requires an inhaler during school hours:
*Asthma Treatment Plan needs to be completed, signed by you and your MD, and returned to the School Nurse with the medication.
If your child is trying out for a Westfield School sponsored Sport:
