My child is entering 9th grade in September. He/she would like to partipate in a Fall Sport. What requirements are necessary?
Your child should register for the sport by picking up forms in the Athletic Office at WHS. In addition to an annual physical, your child needs to complete a Health History Form and a Health History Update Questionnaire for each sport.

FYI - If the annual physical is performed during the summer, the student will be able to sign up for any sport during the school year and use the one physical exam; only the Health History Form and Health History Update Questionnaire would have to be updated for each subsequent sport.
When should I keep my child home from school?
Healthy students make better learners. The Westfield Administrative Regulations as written by the district physician advises that your child stays home if any of these symptoms exist:
Fever 100.0 F or greater. Child must be fever free for 24 hours, without fever-reducing medication to return.
Fever within the last 24 hours
Frequent, hard coughing
Vomiting and/or diarrhea within the past 24 hours.
Unexplained/undiagnosed rash or skin eruptions. Needs physician note to return to school.

When should I return my child to school --- The Westfield Administrative Regulations as written by the district physician advises that that you return your child to school when the following occurs:
Temperature normal for the past 24 hours without fever reducing
medicines
No vomiting for last 24 hours
Child feels well enough to be in school for a full day
If my child needs to take medication (prescription or non-prescription) during school hours or on field trips, what is the procedure that needs to be followed?
Medication must be administered to students in school by the School Nurse.
A medication form should be requested from the school nurse. On this form there is a section for a physician's as well as a parents signature. The physician should include diagnosis, dosage, and route of administration.
Medication must be brought to the Health Office in the original container.
Students who have permission to self administer inhalers or epinephrine must first check with the school nurse for further instructions.
Parents are always allowed to come to the Health Office to administer missed doses of medication when necessary.
