*If you have a returning student and have previously completed final forms, contact the school office at the number listed in Step 4 to have your account reinstated. Then you must update all information and sign.
Check your email to confirm and complete your Final Forms registration. Follow the prompts, add your new student and complete the information requested on each page and then sign at the bottom of each page. Once this step is completed, an email will be sent to the school/guidance office alerting us of a new enrollment.
Final forms must be completed before you come in to complete enrollment.
All of the forms below are required for new enrollments (and for those students returning to Westfall). If able, please print and bring completed forms with you. If not able, forms are available at each school building.
Residency Affidavit II - All grades (Complete ONLY if living with a friend and/or family member - Must be notarized)
Schedule a time to meet with the secretary from the school building(s) you need to complete enrollment. This meeting is done in person and a picture ID will be required.

Valid Photo ID of Parent/Guardian (i.e, driver's license, passport, etc.)
Student Birth Certificate
Student Social Security Card
Court / custody documents (foster placement, grandparent POA, shared parenting, divorce or custody documents)*if applicable
Two Proofs of Residency - Listed on the residency affidavit form
Immunization Records
Special education / gifted records (IEP, 504, WEP, or WAP)*if applicable
Current Grade Card or Progress Report (K-8th); or Final Report Card (incoming 9th grader); or Unofficial Transcript (incoming 10th - 12th grade) HIGH SCHOOL ONLY: Please note that we will not be able to complete a high school schedule for your student(s) without records from their previous school, unless you can provide the document above at the time of enrollment.
