Interested in playing sports at Blendon?
Follow these steps now so that YOU can attend open gyms/tryouts for your season!
Go to Final Forms (https://westerville-oh.finalforms.com) to create a parent account.
Click on "Instructions" for help getting started and then "Add Student" to create a profile.
After you have added your student, connect the sport they want to play to their profile. This will allow coaches to communicate with you.
Upload a current physical, follow the prompts to electronically sign all forms, and when all sections are GREEN your student can try out for sports (provided he/she is also eligible academically).
