Required Forms needed to participate in open gyms/tryouts at Blendon Middle School

• • Sports physical (valid for one year)

  • Emergency Medical Authorization form 

  • Signed Concussion Form

  • Signed Sudden Cardiac Arrest Form

  • Signed Lindsay's Law Parent/Athlete Form

  • Parent AND Athlete electronic signatures in Final Forms

(Spanish Version of Physical Form)

Pay to participate fees and any school related fees, must be paid in full prior to the first scrimmage/competition for each sport.

Eligibility Information

All athletes must meet eligibility requirements established for WCS. Eligibility requirements include passing at least 4 classes and carry a minimum of a 1.75 GPA.  This rolls over from the 4th grading period to the fall for athletes wishing to try out for fall sports.
