Hello and Welcome to 6th grade!
My name is Mrs. Lemons and I will be your nurse in middle school. If you need my help during the school year, ask your teacher so they can call me or send a pass with you. The nurse clinic is located in the front office area.
Parents/Guardians and Students, please read the following guidelines:
· Students entering 6th grade are required to get vaccinated for MCV4 and Tdap before 1st day of school.
· Students are not allowed to carry any form of medication on the bus or in school. Parent/Guardian will need to contact the nurse regarding the need for emergency Epi-pens/inhalers.
· If a medication is required to be taken during school hours, parent/guardian will need to bring it to school office and sign a consent form. Any medication needed in the morning needs to be administered at home.
· Do not call/text parents to pick you up when not feeling well. Ask your teacher to send you to the nurse.
· When calling parents, nurse will call the first number provided to the school in Skyward. Then move down to the next as needed. Emergency contacts are called when parents are unable to be reached.
· Do not come to school when sick. Currently, Western is requiring 10 day quarantine for one symptom. These symptoms include: fever 100.0 or higher, severe headache, cough, shortness of breath/difficulty breathing, sore throat, diarrhea, vomiting, abdominal pain/cramping, congestion or runny nose, loss of taste or smell. Only a Covid test can diagnose Covid since the symptoms can mimic many things.
