Report a Concern

FMS is a school of students and staff that care deeply about one another and want to help each other succeed. As part of this effort, we have created this form to report concerns that a student, staff, or community member may have about an FMS student.

This form will be confidential and will only be seen by the Counselors at Foothills Middle School.
