Fill out this form if you need your ADAP card and/or your Certificate of Enrollment for your driver's license or permit. Make sure that you have your appointment with the Dept. of Driver's services before you fill out the form. Also make sure that it is within a week of your birthdate because the form is only good for 30 days.
You must use your student Google account to fill out the form. Mrs. Stone will call you up to the front office to pick it up.
You will need to get a Parking Permit before driving your car to school. Fill out this form and bring $10 to Mrs. Sanders in the media center to obtain your permit. You must upload a photo of your drivers license and a CURRENT insurance into the form.
