If page 6 is not filled out, the chaperone will not be allowed to give your child ANY OTC medication.
ALL MEDICATION MUST BE WITH YOUR SON/DAUGHTER BEFORE THE DAY OF THE TRIP. YOU CANNOT GET MEDICATION THE MORNING OF THE TRIP FROM THE OFFICE OR ATTENDING NURSE.
ALL MEDICATION WILL BE HELD BY YOUR SON/DAUGHTER'S CHAPERONE.
ANY APPROVED MEDICATION ENOUGH FOR THE THE DURATION OF THE TRIP IN A ZIPLOCK BAG WITH DOSAGE CLEARLY WRITTEN ON AN INDEX CARD OR PIECE OF PAPER (ALL MEDICATIONS WERE LISTED ON THE MEDICAL FORMS AND APPROVED BY THE PHYSICIAN.
