* Complete the packet ASAP - In the packet there are there are some forms that are filled out by the parent. The Administrative Approval form must be completed by a WJH school administrator. The Sports Physical with the Maturity Form must be completed by the student's doctor (if the student has an up-to-date physical in the nurse's office, the student can see the nurse about this). The Sport Skill Evaluation form should be completed by the student's most recent coach.

* Once the packet is reviewed by the Athletic Director, a determination is made if the student meets the criteria to proceed with the Physical Fitness Test. Then, the physical fitness tests are scheduled by the Athletic Department.
