Transition

A successful transition process is based on the student's strengths, preferences and interests and requires collaboration between the school district, student, family and community agencies. Transition planning and services are designed to prepare a student with a disability to achieve his or her post-secondary goals related to living, learning and earning within the community.


ACCES-VR:

Adult Career and Continuing Education Services-Vocational Rehabilitation (ACCES-VR) starts with the presumption that all individuals with disabilities can benefit from vocational rehabilitation services and should have opportunities to work in jobs integrated within their communities. Vocational Rehabilitation Counselors guide individuals through service programs they need to reach their employment goal.

All individuals with disabilities who want to work will have the opportunity to achieve employment and independence.

Once applications are completed you can return it to the School Psychologist and they will be able to assist with the additional documentation required.

Office for People with Developmental Disabilities (OPWDD)

The New York State Office for People With Developmental Disabilities (OPWDD) is responsible for coordinating services for New Yorkers with developmental disabilities, including intellectual disabilities, cerebral palsy, Down syndrome, autism spectrum disorders, Prader-Willi syndrome and other neurological impairments. It provides services directly and through a network of approximately 500 nonprofit service providing agencies, with about 80 percent of services provided by the private nonprofits and 20 percent provided by state-run services.

College Planning

All colleges and universities offer accommodations for students who had IEP and/or Section 504 Plans during their K-12 education. The offices have different names such as "Office of Accommodative Services". Most require the student to apply and provide documentation of the disability.
