WAKE COUNTY PUBLIC SCHOOLS

ATHLETIC POLICIES

1. Only 7th and 8th grade students are allowed to participate in sports in middle school.

2. Students participating on or after the first tryout/practice shall have on file at school a completed and signed Middle School Athletic Participation Form, which includes:

• A valid physical examination

• Certification of accident insurance 

• Athletic participation/parental permission

• Information/Treatment-Consent Authorization 

• Student-Athlete health history

• Assumption of risk

• Filled out and signed concussion statements
