Enrollment Information

After you complete the online form, wait for your base school to contact you with the next steps. Be prepared to come to AFMS with the following:

1. The original copy of your child's birth certificate.

2. Proof of your Wake County address (domicile) in the name of the birth parent or legal custodian, which must be one of the following:

   • A current water, gas, or electric bill within 30 days of the due date,

   • A newly signed lease that is no more than 60 days old,

   • A Settlement statement that is no more than 60 days old

     • Capped schools

       • An Offer to Purchase Contract is never accepted as proof of residency for enrollment or pre-enrollment at a capped school.

3. A photo ID of the birth parent or legal custodian

4. Your child's immunization record

5. Any custody documents
