If you have not participated in a sport at Peirce, you will need a PIAA PHYISCAL performed by a doctor. You can get these at your doctor's office, emergency clinics, or CVS/Rite Aid minute clinics.

FamilyID is a secure registration platform that provides you with an easy, user-friendly way to complete the PIAA required CIPPE form to participate in athletics, When you register through FamilyID, the system keeps track of your information in your FamilyID profile. You enter your information only once for each family member for multiple uses and multiple programs.

This form is for any athlete that participated in a fall or winter sport at Peirce. It will need to be signed by both the athlete and the parent.
