Please submit this form to request permission to use any part of the building and/or to add items to our Google Calendar. This Form will be sent to administration for approval.  You will receive an acknowledgment .  You will receive another email  when request is approved/denied.
Discipline Report - Submit for discipline issues

This form is to be submitted and approved by the Principal, Superintendent, and the Board of Education BEFORE any orders are placed or final arrangements are made.   You will receive an acknowledgment and another email when approved/denied.  If approved a PDF of the form will be attached.
Submit to request any keys in the building.
Submit to request a post to one of our social media platforms
Please submit this form before leave is taken or immediately upon return to work in the case of illness or emergency.  Non-compliance may result in these days of absence being deducted from your pay.  The form will be sent to administration for approval.  You will receive an acknowledgment and another email when approved/denied.   If approved a PDF of the form will be attached.

Submit for ALL purchases for the 2023-2024 school year.  This form is sent to finance for input into MUNIS where administration can view and approve.
Submit to document information on student situations.

Submit after returning from Professional Development.  Have receipts ready to upload.
Submit for SCHOOL DAY  trips involving buses.  This form submits to AD (athletic trip) or Principal (Education/Extra-Curricular) for approval.  If approved form is automatically sent to transportation to reserve buses.  Information is also sent to our event calendar.  You will receive an email when approved/denied.  If approved the email will contain a copy of the form.
