Please be sure to complete and sign-up for the following items:

Contact Form - This is used in case of any emergencies. This must be completed before band camp!

Charms - This is used to track your financial records and for email communication.

The Band App - This is our text messaging service.

Wayne Medical Form - This form is required by the MSD Wayne in order to participate. This must be completed before band camp!

Physical Form - This form is a requirement for participation. If you have one on file from another sport, you do not need an updated physical UNLESS it becomes more than a year old during the season. Please notify us if you have a physical form on file with the school.
