**must use the version revised as of 7/19**

**must be completed for any out-of-state OR overnight trips

**must complete the date of accident and turn into health aide

**after completing accident report, give this to student/parent before leaving facility the date of accident

**any WCPS student-athlete is subject to random drug testing during the season of participation

**must be notarized...required for athletic trips in which parents will be transporting and/or providing lodging for child

**must be submitted to mike.wilson@warren.kyschools.us for approval
