The FACTS is a two-page interview used by school personnel who are building behavior support plans. The FACTS is intended to be an efficient strategy for initial functional behavioral assessment. The FACTS is completed by people (teachers, family, clinicians) who know the student best, and it is used to either build behavior support plans, or guide more complete functional assessment efforts.
The Motivation Assessment Scale (MAS) is a questionnaire designed to identify those situations in which an individual is likely to behave in certain ways. From this information, more informed decisions can be made concerning the selection of appropriate reinforcers and treatments.
This worksheet is designed to assist multidisciplinary teams in the identification of behavioral skill andperformance deficits after the first stage of a functional behavior assessment has resulted in a presumed function for a target behavior.
This worksheet assists the student to identify specific behaviors that are resulting in problems in the school or classroom. Making suggestions or paraphrasing statements can help the student clarify his or her ideas.
