Please complete (with Physician) if your child will require medication to be given in school. Prescription medications and all over the counter medications require an order from a physician and parental permission as per School Policy and State Law. Not required for food allergy emergency meds or asthma meds (see those required forms below).
Please complete if your child has any medical concerns that need to be brought to the attention of the school nurse.
Please complete (with Physician) if your child will require an Epi-Pen and/or Benadryl to be given in school. Please complete the Epi-Pen Delegate Form as well.
Please complete and return with Food Allergy Action Plan.
Please complete (with Physician) if your child will need an inhaler or nebulizer in school
