Here's how:

1.  Download and complete the Out-of-District Professional Development PAC Request form (complete the top box on the form for "Conference, Seminar or Convention").

2.  Submit the form by the end of the 2023/2024 school year to transcripts@madison.k12.wi.us

3.  Attach a copy of the schedule for the Summit to your form as proof of attendance.
