You must provide one document from List A 

OR 

You must provide one document from List B AND one document from List C

Select your document(s) referencing the list to the right, and make a photocopy of the front and back of each document to upload into GHO. When making your photocopy ensure the dates, document numbers, and your photo is clearly visible.

Your authorized representative will use your document(s) to complete Section Two of your Form I-9 and the I-9 Affidavit. 

An authorized representative can be any one such as a family member, friend, roommate, neighbor, etc. as long as they are over the age of 18.
