CHANGE IN IMMUNIZATION REQUIREMENTS
The Wisconsin Student Immunization Law requires that all students through grade 12 meet a minimum number of required immunizations prior to school entrance.
PLEASE BE AWARE: The Meningococcal vaccine is required for 7th graders (one dose) and 12th graders (two doses). 12th graders who received their first dose of Meningococcal ACWY containing vaccine at age 16 or older are only required to have one dose. Also, parental reporting of chickenpox no longer qualifies as an exception to the varicella vaccine and will require a diagnosis from a health care provider.
Waivers to vaccinations are still permitted for religious, medical or personal conviction. The Waiver section can be found on Step 4 of the Student Immunization Record.

*Please note that parents have the right to waive immunizations for their child due to the following reasons: health, religion, or personal conviction. If you choose to waive your child's immunizations, a signed form indicating that you wish to waive your child's vaccinations still needs to be returned to the school in order to be compliant with State Requirements. Keep in mind that in the event of an outbreak of a vaccine preventable disease, students with waivers may be excluded from school until the outbreak subsides.
