Chromebook is Damaged
HS and MS students bring damaged Chromebook to their main office. For ES students, teachers can bring it to their main office.
Students do not bring it to the technology director or just send it to the tech office.
Please fill out a Chromebook Repair Request form. A paper copy must accompany the damaged device.
Make sure you know your Username, Password, Asset Tag number of the device.

Notify the office that your device needs repaired, and you'd like a loaner. They'll get one ready for you. If you don't need a loaner, that's ok, too. Just be sure you're able to work throughout the day without one.
If you do take a loaner, it must be returned at the end of the day. It cannot be taken home.
The device will be repaired and returned to the student.
The paperwork will remain on file and any cost associated with the repairs will be added.
As a student at Waterloo Local School District, you will have available to you a device that can connect to many of the resources you'll use for instruction. Typically, this device will be a Chromebook (of various models). As such, on issue, you will be bound by the following Device Specific User Agreement:
THIS AGREEMENT APPLIES TO BOTH THE PARENT AND STUDENT USER OF ANY TECHNOLOGY EQUIPMENT OWNED AND DISTRIBUTED OR ISSUED BY WATERLOO LOCAL SCHOOL DISTRICT.
BY ACCEPTING THE EQUIPMENT ISSUE, EACH FAMILY / PARENT / GUARDIAN AGREES TO BE FINANCIALLY LIABLE FOR ANY LOSS OR DAMAGES TO THE EQUIPMENT (VALUE OF DEVICE, POWER CORD) FOR THE ENTIRE PERIOD OF THE ISSUE.
THIS INCLUDES ANY DAMAGE TO THE DEVICE - HARDWARE, SOFTWARE, POWER SUPPLY, ETC. THE DISTRICT IS NOT ABLE TO PROVIDE ACCESS TO THE INTERNET VIA HOTSPOT / WIFI TECHNOLOGY AT HOME.
BY ACCEPTING A DEVICE ISSUE, YOU - THE PARENT OR GUARDIAN OF THE STUDENT TAKE FULL RESPONSIBILITY FOR THE DEVICE, AND GUARANTEE ITS SAFEKEEPING, PROPER USE, AND SECURE STORAGE AT ALL TIMES AND UNTIL THE DEVICE HAS BEEN RETURNED TO THE SCHOOL DISTRICT. IF THE DEVICE IS RETURNED WITH ANY DAMAGE, YOU WILL BE FINANCIALLY LIABLE. DETERMINATION OF DAMAGE WILL REST WITH THE WATERLOO TECHNOLOGY DEPARTMENT.
From time to time, a device may sustain some damage requiring either repair, or complete replacement. This why each student will be charged a Technology Fee to cover device insurance supplied by the district. During normal use, the expectation is that the student, with proper use and care, should be able to sustain few, if any damages during normal, responsible, use of the device.
IF THE DEVICE IS LOST OR STOLEN, A POLICE REPORT MUST BE FILED BY THE PARENT/GUARDIAN, AND THAT REPORT SUBMITTED TO WATERLOO LOCAL SCHOOL DISTRICT.
IF THE DEVICE IS NOT RETURNED WITHOUT A POLICE REPORT, THE PARENT/GUARDIAN WILL BE FINANCIALLY LIABLE FOR FULL REPLACEMENT COST.
Any fees assessed for repairs will be placed on the student fee account, and must be paid immediately.
Do NOT attempt to repair the device yourself or take it to a repair place for service.
