DATA PROTECTION: We are fully HIPAA compliant. In order to provide you with

Health care we must keep your medical records. Our office will

ensure that your medical records and your personal data remain confidential. Please inform us of any change in your personal data – telephone number, address, insurance information.

EMERGENCIES : In case of emergency, patients will be seen usually the same day, by a doctor not necessarily of your choice. But please call before coming to the office. If you are too sick to come to office or if the office is closed, please call 911 and go to the nearest emergency room and inform the emergency room staff that Dr. Sinha is your doctor and Emergency Room will contact us.

PRESCRIPTIONS : 48 hours notice for repeat prescriptions are needed. Please avoid asking for refills on week ends and holidays. Call your pharmacy and request the pharmacy to call us during week days work hours, 2 to 3 days before you run out of your medicines.

TEST RESULTS : Results of your blood tests , X-rays etc. are given by your doctor unless asked by doctors to be given by our staff, only to the patient.

CHILDREN / ADOLESCENTS CARE and FAMILY PLANNING – Please consult your appropriate physician. We do not provide these services.

PAP SMEARS : Please consult your Gynecologist for yearly PAP Smear.

HEALTH SCREEN : You may request yearly routine health Check up. Please check your health insurance policy to see if this service is covered. Some health insurance policy do not cover routine yearly visit. Cost depends up on your age and the needed tests.

FLU VACCINE : We provide yearly Flu (Influenza) Vaccine between end September and end December. We encourage you to take the vaccination to prevent the attack of Flu. DO NOT take this vaccine if you are allergic to Chicken or the egg. Some insurance companies do not cover the FLU Vaccine in which case you will be billed for this service.

PNEUMONIA VAC : Pneumonia vaccine is available. Please discuss with your doctor to find out if you need it.

OTHER VACCINES : We will provide other vaccines on demand and on payment. Most Insurance companies pay less than the cost of the vaccine and hence we require payment from you for other vaccines.

EMPLOYMENT : Your Insurance does not pay for employment physicals.

PHYSICALS : Payment for this service is your responsibility.

CO-PAYMENT : Co payment is your responsibility and is expected to be paid at the time of service. Please pay your co payment.

We also provide care for Motor Vehicle Accidents injuries and injuries from work . Please speak to your work Supervisor for authorization to treat you.
