This form should be used for students to either renew or create a new club. It needs to be submitted to ASB with bylaws attached for approval.
At least 2 weeks prior to the fundraiser, paperwork needs to be submitted to request the approval for a fundraiser. Upon approval, the club commissioner will notify you of its approval.
