 Required paperwork:

• Child's original birth certificate

• Guardian legal ID/License/Passport

• Residency Documentation

• Proof of Residency (one of the following)

  • current property tax bill

  • lease with children's names listed on the lease

  • fully signed Purchase and Sale Agreement (P&S) provided that the occupancy date occurs within 45 days of enrollment

• Proof of Occupancy (one of the following)

  • current utility bill

  • excise tax bill

  • recent bill within 45 days showing Natick address and name of parent/guardian

• Immunization Certificate and Exam Certificate

  • it is acceptable if your doctor uses a different form to complete this information, as long as our required information is included and the record is complete

• IEP/504, if applicable

• English Language Learner Transcript/ACESS and WIDA Scores, if applicable

• Custody Documents/Caregiver Affidavit, if applicable

PLEASE NOTE:  Students cannot be enrolled until all information is reviewed and verified.
