To recommend a student, they will need a recommendation form filled out by you. You can either fill out the form online or fill one out physically and email it or put it in my box outside of the CIS room. If you have any questions about if a child might be eligible feel free to email me.

To begin services, each child (even if siblings) will need a parent consent form. You may send one home with the student when recommending them or I will provide them with one once I follow up with them.
