Tryout Application 

Pre-Participation Physical Evaluation Form

Print off this form, complete it, and bring a hard copy to Day 1 of the tryout clinic on Monday, April 7th. This form has 5 parts:

1. Medical history

2. Physical examination form

3. Clearance form

4. Consent for athletic participation and medical care

5. Athlete with special needs supplemental form (only required if applicable to the student)

If you are unable to print this at home, hard copies of this form will be available in the WVMS Office.

Concussion Form

Print off this form, complete it, and bring a hard copy to Day 1 of the tryout clinic on Monday, April 7th. If you are unable to print this at home, hard copies will be available in the WVMS Office.
