Welcome to the WELL-BEING SERVICES patient information page. This section is designed to provide individuals receiving Community Health Worker (CHW) services with important information about available support, patient rights, privacy protections, and service expectations. Our goal is to deliver respectful, person-centered assistance that helps you navigate healthcare, connect with community resources, and improve overall well-being.
On this page, you can review your Patient Bill of Rights, learn about confidentiality and non-discrimination policies, understand how CHW services work, and complete the intake form to begin receiving support. We encourage you to review this information carefully and contact us if you have any questions.
INTAKE FORM
To begin receiving Community Health Worker (CHW) services, please complete our secure Patient Intake Form. This form allows us to gather basic information about your needs, Nevada Medicaid coverage, and preferred communication methods so we can provide appropriate support and connect you with available resources. Completing the form is voluntary, and all information submitted will be kept confidential in accordance with HIPAA and applicable privacy laws. Once your form is reviewed, a member of our team may contact you to discuss next steps.

PATIENT BILL OF RIGHTS
WELL-BEING SERVICES is committed to providing respectful, culturally responsive, and person-centered Community Health Worker (CHW) services to Nevada Medicaid recipients. As a patient, you have the right to be treated with dignity and respect, receive services without discrimination, and participate in decisions about your care and goals. You have the right to receive clear information about CHW services, including care coordination, health education, resource navigation, and support services. You may refuse services at any time, request changes to your service plan, and choose whether to participate in recommended activities. CHW services are voluntary and designed to support your individual needs while working in coordination with your healthcare providers and care team.
Your privacy is important to us. In accordance with the Health Insurance Portability and Accountability Act (HIPAA), your personal and health information will be kept confidential and used only for treatment, payment, and healthcare operations unless otherwise permitted or required by law. Community Health Workers may communicate with your healthcare providers, care managers, or community partners only as necessary to support your care and in accordance with your consent and applicable privacy laws. You have the right to request access to your records, request corrections, receive a copy of our Notice of Privacy Practices, and ask questions about how your Protected Health Information (PHI) is used or shared.
PATIENT RESPONSIBILITIES
To help us provide effective services, patients are expected to:
Provide accurate and complete information to the best of their ability
Participate in developing and following their service goals when they choose to engage
Notify Well-Being Services if contact information changes
Treat staff with respect and professionalism
Keep scheduled appointments or provide reasonable notice to reschedule
Inform staff if they no longer wish to receive CHW services
Follow recommended safety guidelines during in-person or virtual visits
NON-DISCRIMINATION & ACCESSIBILITY
WELL-BEING SERVICES complies with applicable federal and state civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, sex, gender identity, sexual orientation, religion, or any other protected status. Services are available to all eligible Nevada Medicaid recipients regardless of background.
We provide reasonable accommodations for individuals with disabilities in accordance with the Americans with Disabilities Act (ADA). Auxiliary aids and services, such as interpreters, large-print materials, and other accommodations, are available upon request at no cost.
Language assistance services are also available free of charge for individuals with limited English proficiency. This includes qualified interpreters and translated materials when available. Please contact us if you need language assistance.
CONSENT FOR COMMUNITY HEALTH WORKER (CHW) COORDINATION
By receiving Community Health Worker services, you acknowledge that CHWs may coordinate with your healthcare providers, care managers, Medicaid Managed Care Organization, and relevant community service organizations to support your care and service goals. This coordination may include sharing limited information necessary for referrals, care coordination, resource navigation, and follow-up services, in accordance with HIPAA and applicable privacy laws. You may revoke or limit this consent at any time by notifying WELL-BEING SERVICES in writing.
GRIEVANCE / COMPLAINT CONTACT INFORMATION

EMERGENCY DISCLAIMER
Community Health Worker (CHW) services provided by WELL-BEING SERVICES are non-emergency support services. CHWs do not provide medical care, crisis intervention, or emergency response. If you are experiencing a medical emergency, mental health crisis, or situation that requires immediate assistance, call 911 immediately or go to the nearest emergency room.
If you are experiencing a behavioral health or emotional crisis, you may also contact the 988 Suicide & Crisis Lifeline by dialing 988 or visiting https://988lifeline.org for immediate support.
Community Health Workers are available to assist with care coordination, health education, and resource navigation, but they cannot respond to urgent or emergency situations. If you contact Well-Being Services with an urgent issue, you may be directed to appropriate emergency services.
AFTER-HOURS DISCLAIMER
WELL-BEING SERVICES does not provide 24-hour emergency coverage. Messages received outside of normal business hours, on weekends, or on holidays will be returned during the next business day. If you need immediate assistance outside of business hours, please contact your healthcare provider, call 911, or seek care at the nearest urgent care center or emergency department.
RESPONSE TIME DISCLAIMER
WELL-BEING SERVICES strives to respond to patient calls, emails, and messages within a reasonable timeframe, typically within one business day. Response times may vary based on staff availability, volume of requests, and holidays. CHW services should not be used for urgent or time-sensitive medical needs.
NO MEDICAL ADVICE DISCLAIMER
Community Health Workers do not diagnose conditions, prescribe medications, or provide medical advice. Information provided by CHWs is for educational and care coordination purposes only and should not replace guidance from a licensed healthcare provider. Patients should consult their physician or qualified healthcare professional for medical concerns.
SUPERVISION DISCLAIMER
Community Health Workers operate within their scope of practice and work under the direction and/or supervision of qualified licensed healthcare professionals as required by Nevada Medicaid program guidelines. CHWs provide support services such as care coordination, health education, and resource navigation, and do not independently provide clinical or medical services.
