Our agency currently provides quality outpatient services from Intensive In-Home Services(IIS), Applied Behavior Analysis (ABA), Behavioral assistance and mentoring Services (BA), Individual Support Services (ISS), Respite Services, Community Based Supports, Community Inclusion Services, Intensive In-Community Services (IIC) as well as Supports Coordination Services to I/DD eligible adults 21+ over.
INTENSIVE IN-HOME SERVICES
In-home services are provided on an outreach basis to work with individuals and their families in the home and community. They are designed to avert treatment in residential or inpatient settings or to facilitate the earlier return of individuals receiving inpatient or residential care. Services are multi-faceted and include therapy, supportive counselling, skills training and facilitation of access to other needed services and supports. Some in-home approaches are limited to short-term, intensive approaches with the goal of stabilising and connecting the individual and family with ongoing services within a few weeks. We also provide longer-term interventions with the service intensity varying with clinical needs.
BEHAVIORAL ASSISTANCE & MENTORING SERVICES
Our Behavioral assistance program is designed to augment Partnership for Children Initiative services through the use of trained behavioral Assistants working directly with children and adolescents and their families in a home, school/work, or other community settings to carry out elements of a treatment or service plans. Services are generally time limited, focused on specific goals used to aide in the transition between levels of care or to facilitate adjustment to developmental tasks.
(ISS) INDIVIDUAL SUPPORT SERVICES
ISS is geared toward teaching the youth and family necessary skills as well as educate them to successfully implement important life skills. The service addresses adaptive behaviors and skill development for Activities of Daily Living, also called ADL'S The duration of ISS services lasts anywhere from 2 to 12 months, and up to 10 hours per week depending on the severity of the case as as the needs of the family.
(IIC) INTENSIVE IN-COMMUNITY SERVICES
IIC is an analytical, focuses, goal-oriented, and needs-based clinical intervention that addresses emotional and behavioral challenges of youth with moderate to high needs. These services can be provided in the youth’s home, or in amenable community location by a Master’s/Licensed Behavior Health Clinician, and is intended to stabilise and support the youth and family. The IIC Clinician also works with a Behavioral Assistant to create the appropriate plan to reduce behavioral challenges faced by the client.
SUPPORTS COORDINATION
Supports Coordination is the general but detailed service of advocating, identifying, developing, coordinating and accessing supports and services on behalf of individuals or assisting individuals and their families to access supports and multiple services on their own. In order to be eligible for this services one must be I/DD eligible and should be an adult 21+.
RESPITE SERVICES
Respite services provides family caregivers with a short period of rest or relief by arranging alternate care giving for the family member with a developmental disability. Respite also can be provided when the family caregiver is temporarily absent or disabled for any reason, such as for a short period of hospitalisation.
COMMUNITY INCLUSIONS
These services are provided outside of a participant’s home that support and assist participants in educational, enrichment or recreational activities as outlined in his/her Service Plan that are intended to enhance inclusion in the community.
